For what it's worth Matchless, you did the right thing in RIDDOR reporting it if that's what you did. If someone carked it due to CO poisoning or a house fire and you knew about it you would never forgive yourself.

The HSE will more than likely do nothing to be honest but at least you can say "I told you so", if something does come of it, eh?
At least you passed the buck.

Thanks for the warming words, much appreciated. Yes indeed, i have reported it. I did not like the fact how it was installed. Even though Id Be wrong or over sensitive I cared about the safety my customer.

I wouldn't say you were wrong.
We all know that the products of combustion from fossil fuels or wood Contain CO and CO2. Both are dangerous. If they don't make it up the flue for any reason, there could easily be an injury or fatality.
In this day and age were blame is prevalent it is always best to err on the side of caution.
 
Found this, I see this sort of thing often but not my work and not my problem. Customer has been advised, that’s all. :)

What a mess that is. Is it a working gas appliance attached to it?

Worse, in some ways! It has an oil boiler attached to it which is an old very inefficient boiler, so gases are well in excess of 300 degrees and you will notice the terminal end piece has come off. (Gases supposed to go out side vents on that flue)
That oil tank is getting some heat!
Regs on new work are oil tanks 1.8m at least from any terminal
