Chemical cleaners. I use either Kilrock de-scaler or some stuff called Ataka. I think they are both based on some form of weak acid (acetic or possibly formic). They give off unpleasant smell / fumes when working, so you need ventilation. There is some more domestic stuff called Vakal, but I've never used it. I have heard people use vinegar, but again have never tried it.
 
Sainsburys stock something called "lime lite" which I found works well without being agressive like a descaler. Kilrock K and other descalers can stain metal enclosures and tends to take the plating of waste fittings and taps.

Domestos Grot Buster is handy for bleaching discoloured or stained grout.
