Apologies for the poor drawing but I'm tired, this is a y plan sealed system. As you can see the hot water is not the last tee into the circuit, the issues with this are the central heating will not return effectively resulting in very poor performance on the rads. It's like it's air locked or blocked. 100% no blockage in pipework, bypass check, 3 port checked, pump checked. I'm really just interested in whether the pipe layout would be affecting it?

I've only drawn the return pipework to simplify it

I'm guessing the pump is near the boiler? That 't' in the top right of your pic wont help one bit
 
The cylinder looks like the cylinder is on the last tee? I think saying last tee is wrong maybe we should say cylinder and rads teed together once
 
Put a gate valve on return from cyl and bring all your rads to a common return . In effect two units

looks ok to me from that drawing the cylinder is the last tee its where it is teed .
 
just out of intrest force the boiler into full rate using the engineers code and gas rate it to make sure its not modulating down - check the d settings to make sure its set to the correct kw rating and not been range rated down .

check d0 is set to max kw
check d5 flow temp is set to 75 -82
use d3 to see what temp flow is getting to
