Are you required to fill out servicing sheets when you service a gas boiler?

My procedure:

Service boiler as per manufacturers instructions(fga & all the usual stuff)
Check flue termination
Bleed radiators
Visual inspection of installation
Tightness test(keep print out on file)
Raise any NCS verbally, send written list of ncs with invoice.
Issue AR/ID if ever required.

The firm I used to work for never did a ''servicing sheet'' after completing a boiler service. I was just wondering because I'am in college doing my oftec and the lecturer recons I should be doing servicing sheets with every boiler I do gas or oil; he also thinks I should have a 5 million page coshh book in my van too though..
 
looks more efficient and formal and you also get a record of your work as proof of proper compliance and that where my printout gets attached, customer has it written on their copy

When it was CORGI I know you didnt have to I am not sure if gas safe are the same. It is best practice to do it as then you have something to come back on if needed and it only takes a minute to fill one out
