Your system was installed in 09 has it been serviced and checked each year ? there is definitely a serious problem which needs sorting out this is a tell tale sign get a G3 engineer to do the repairs. Kop

There is no way the system has been serviced. The main PRV has zero marks on it to evidence it being pulled apart for the filter to be cleaned.
I'd also estimate the pressure vessel has failed too. Why, evidence around the tundish and it looks to me like there is too much weight in the vessel cause it to lean.
In my opinion, first port of call would be G3 to get it properly serviced PLUS send teh pipe section off to manu
